Pre-pandemic death certificate copies may be obtained in person, online or through the mail via the Town Clerk/Vital Records office in the town where the death occurred. Post-pandemic death certificate copies may be obtained in person at any town clerk's office in the state.

Complete the Application for copy of Death Certificate available on the respective town clerk website and mail it or bring it in person to the vital records office of the town or city where the death occurred. Certified copies are $20/each. Most town clerks do not accept personal checks.
