Forms to Complete

To ensure every cremation is carried out without delays and in compliance with your wishes and the law, we need certain information and your authorization to proceed.

Below are the relevant legal forms. Please complete and sign these forms after you have arranged the cremation, then return them to us at the Cremation Society of New England as soon as possible. Without them, we cannot carry out the cremation.

Please note that delays in returning these forms may result in additional fees for extended refrigeration. We hold remains for up to 72 hours as part of our basic services. After that time has passed, there is a fee of $95 per day or portion thereof for extended refrigeration.

There are several forms that we need to facilitate a cremation. Connecticut law requires hard signatures. We make the process simple:

1. Download our Simple Form Packet

2. Complete the information sheet on the form on the second page - this will populate the other pages in the packet with the appropriate information.

3. Print the packet and sign where indicated on each page.

4. Send the forms to our office.
