To ensure every cremation is carried out without delays and in compliance with your wishes and the law, we need certain information and your authorization to proceed.

Below are the relevant legal forms. These forms are also available by using the "Arrange a Cremation" button located above [recommended], which allows you to digitally submit them.  To print and submit them via traditional mail, you may use the attachments below.  Without them, we cannot carry out the cremation.

Please note that delays in returning these forms may result in additional fees for extended refrigeration. We hold remains for up to 72 hours as part of our basic services. After that time has passed, there is a fee of $95 per day or portion thereof for extended refrigeration.
